  On this 17th day of March, A. D. one thousand eight hundred eighty four personally
appeared before me, Clerk of the County Court, a court of record within and for the
county and State aforesaid, Stephen M. Warner, aged 48 years, a resident of the county of  Bath, State of Kentucky, who, being duly sworn according to law, declares that he is the identical Stephen M. Warner, who was ENROLLED on the 10th day of August, 1862, in Company B of the 10th regiment of Kentucky Cavalry commanded by Captain Charles S. Rogers, and was honorably DISCHARGED at Maysville, Kentucky on the 17th day of September, 1863; that his personal description is as follows: Age, 27 years; height, 5 feet 7 inches; complexion, light; hair, black; eyes, hazel.  That while a member of the organization aforesaid, in the service and in the line of duty near Sharpsburg, in the State of Kentucky on or about the 20th day of November, 1862, he was disabled in the following manner his horse falling and injuring his testicles by falling on him causing the loss of one of his testicles also causing rheumatism.

That he was treated in hospitals as follows: treated by Regimental Sergeant on or about
15th December 1862.

That he has not been employed in the military or naval service otherwise as stated above.

That since leaving the service this applicant has resided in the County of Bath in the State of Kentucky, and his occupation has been that of a Farmer.

That prior to his entry into the service above named was a man of good, sound physical
health, being when enrolled a farmer.  That he is now partly disabled from obtaining his
subsistence by manual labor by reason of his injuries, above described, received in the
service of the United States; and he therefore makes this declaration for the purpose of
being placed on the invalid pension-roll of the United States.

  He hereby appoints Robert M. Dore of Bath county, State of Kentucky, his true and
lawful attorney to prosecute his claim.  That he has not received or applied for a pension.
That his Post Office address is Marshall, county of Bath, State of Kentucky.

Sir:
     I have the honor to request that you will furnish from the records of the War
Department a full report as to the service, disability, and hospital treatment of Stephen M. Warner, who, it is claimed, enlisted August 10, 1862, and served as Sergeant in Company B, 10th Regiment, Kentucky Cavalry, he was disabled by Rheumatism and the loss of one testicle.  Alleged as the result of a horse falling on him near Sharpsburg, Kentucky about November 20, 1862 and was treated in the hospitals of which the names, locations, and  dates of treatment are as follows: Treated by the Regimental Surgeon about December 15, 1862.  Please examine the Regimental records.

In the matter of the original invalid pension claim no. 508890 of Stephen M. Warner late
Sergeant of Company b, 10th Regiment Kentucky Volunteer Cavalry on this 12th day of
May, A. D. 1884, personally appears before me Clerk of Bath county circuit court in and for the county did authorize to administer oaths William Otis late Private of Company D, 14th Regiment Kentucky Volunteer Cavalry and a resident of Marshall, Bath county, and the State of Kentucky Who's Post Office address is Marshall, Bath county, Kentucky.  Well known to me to be reputable and entitled to credit and who being duly sworn declares in the relation to the aforesaid case as follows:  I have been aquatinted with Stephen M. Warner 35 years, have resided within one half mile of the aforesaid claimant for the period of eighteen years, he was a stout and hearty man before enlistment and free from alleged disabilities loss of testicle and rheumatism as far as I knew I saw him the claimant while a member of Company b, 10th Regiment Kentucky Volunteer Cavalry at Mt. Sterling, Kentucky in the fall of 1862.  I stayed one or two nights with him the claimant in his tent he was suffering from injuries of rheumatism he was down with his back and hips and could not walk and at Maysville, Kentucky September 1863 he was suffering with rheumatism I had personal knowledge of the fact he was limping around with a stick.  As Sergeant I seen the claimant continuously ever since his discharge from the Army and he has been suffering with rheumatism.  I have person knowledge of the fact as I live in seeing distance of his house 18 years.  I believe he is disabled about one third of his time from following his real occupation on account of rheumatism or some complaint which I believe he contracted in the Army as I have seen him suffering the same in the Army and he further declares that he has no interest in said case and is not concerned in it's prosecution.

SWORN STATEMENT OF GEORGE W. GRIFFEN

  In the matter of the original invalid pension claim no. 508.890 of Stephen M. Warner,
late Sargent of Co. B, 10th regiment Kentucky Vol. Cavalry personally appeared before
me of the aforesaid County and State George W. Griffin a resident of Olympia, Bath
County, Kentucky, who's post office Olympia, Bath County, Kentucky well know to me to reputable and entitled to credit and was duly sworn according to law and he declares in relation to the aforesaid case as follows.
   I have known the claimant Stephen M. Warner for 30 years I was a member of hi
Company B, 10th Regiment, Kentucky Volunteer Cavalry and on or about the November 1862 the aforesaid claimant because disabled by the fall of his horse while out on scout.  He was a stout hearty man previous to enlistment.  I never heard him complain and after the injury from fall of his horse I heard him complaining of his testicles and rheumatism.  I had seen him shortly after the injury unable to get up after being down without the help of comrades.

He further declares that he has no interest in the aforesaid claim nor concerned in its
prosecution.  George W. Griffin left his mark on the document.  Hi mark was witnessed by G. W.  Staton and H. Stull.

SWORN STATEMENT OF JAMES L. NEALIS
State of Kentucky

   In the matter of original invalid pension claim no. 508.980 of Stephen M. Warner late
Sargent of Company B, 10th Regiment Kentucky Cavalry Volunteers.  On 25th day of
January 1887 personally appeared before me Justice Peace James L. Nealis 61 years of age and a resident of Bath county and state of Kentucky who's post office address is
Owingsville, Bath County, Kentucky and who is reputable and entitled to credit and who
is duly sworn according to law and states in regards to the aforesaid case as follows
   I have been acquainted with claimant for the period of twelve consecutive years.  I first saw him in the army a soldier.  The next time I seen claimant was at home since the war.  This was in the year of 1873.  I further state at this time claimed was suffering with
rheumatism was complaining with acute pain.  I further state I have been personally
acquainted and lived a close neighbor to claimant and a distance of 1/2 mile.  I further
state I have seen claimant about once a week for the period of twelve consecutive years
and I further state I have found claimant always disabled with rheumatism.  I further state that claimant rented land from me in the year of 1885 and I further state that claimant was greatly afflicted by rheumatism while tending corn for me.  At this time I have examined claimant myself at the present time and I further state I find claimants muscles has left him and his joints is partly disabled by rheumatism.  I further state I believe claimant totally disabled from performing labor further state that I have personal knowledge of these facts to which I hereby testify to and I have no interest in said case and I am not concerned in its prosecution.

SWORN STATEMENT OF JAMES W. GONE
GENERAL AFFIDAVIT
State of Kentucky, County of Montgomery
in the matter of Stephen M. Warner for invalid pension Company B 10th Regiment
Kentucky Cavalry on this 11th day of February A.D. 1890 personally appeared before me County Court Clerk in and for the aforesaid county duly authorized to administer oath,  James W. Gone, aged 67 years a resident of Sardis in the county of Mason and State of Kentucky who being duly sworn, declared in relation to aforesaid case.
    I was a teamster in Company B 10th Regiment of Kentucky Volunteer Cavalry of 1861 and knew Stephen M. Warner who was a member of said company and I know at
Danville, Kentucky about January 1863 said woman had rheumatism and when we were
camped near Owingsville in Bath County, Kentucky in May following he was afflicted
with rheumatism and he complained of it a good deal till he was discharged and I have
lived joining farms to him for about three or four more years after he was discharged to
wit 1864 1865 1866 and 1867 and he complained of rheumatism all this time.  The reason
I know he had rheumatism while in service I messed with him sometimes.
    He further declares that he has no interest in said case and is not concerned in its
prosecution.

   In the matter of original pension of Stephen M. Warner Sargent Company B, 10th
Regiment Kentucky Volunteers on this 18th day of July A.D. 1895 personally appeared
before me a County Court Clerk in and for said county and state Stephen M. Warner
claimant who's residence is Montgomery County and State of Kentucky and who's post
office address is Mt. Sterling, Kentucky and a person of repute and entitled to credit and whom being duly sworn according to law declares in relation to the aforesaid case as
follows.
   I am the identical Stephen M. Warner aforesaid and I select to take my pension under
the general law as I believe that I am entitled to a greater pension under the general law
than I an now drawing under the act of June 27, 1894 and for rheumatism and its results
which was contracted in the United States Service and in line of duty at Lexington,
Kentucky about 25th of October 1862 and with out any fault of mine and being poorly
prepared for shelter having no tents at the time camped upon the open field and covered
with snow.  Rheumatism had just set upon me i felt numbness and acking of limb and
muscles and about this time I was ordered out upon scout.  I was in charge having only
three men under my command and while out and near Sharpsburg Kentucky my horse fell falling upon me injuring my testicles.  The three men that was with me was David Kilne, Berry Shoultz and Edward Denton.  Shoultz is deceased and I don't know where about of the other two.  Now I have proven rheumatism origin in the service and I am a poor man and of a family.  I have paid out a good deal of money looking up testimony and I am unable to procure any more evidence.  I supplement this affidavit to my original claim and request the commissioner to adjudicate my claim for rheumatism and order me before the nearest and most convenient board for examination.

In the matter of Stephen M. Warner for invalid pension Company B. 10th Regiment
Kentucky Cavalry.
On this 15th day of July A. D. 1899, personally appeared before me County Court Clerk
in and for the aforesaid County duly authorized to administer oaths, Richard M. Trimble
aged 51 years, a resident of Spenser PO in the County of Montgomery and State of
Kentucky whose post office address is Spencer, Kentucky, well known to me to be
reputable and entitled to credit, and who, being duly sworn, declared in relation to
aforesaid case as follows:  I was a comrade of Stephen M. Warner in Company B, 10th
Regiment of Kentucky Cavalry and known that said Warner had a spell of rheumatism in June 1863 while the regiment was camped Roe's Run near Owingsville in Bath County, Kentucky.  He was confined to his tent for some two weeks or more and was treated by Dr. Fleming the surgeon of the regiment.  He complained of his arm and shoulders and he still complained of rheumatism till his discharge in September 1863.

He further declare that he has no interest in said case and am not concerned in its
prosecution.

   On this 3rd day of June, 1925, before me, the undersigned, personally appeared Stephen M. Warner, who makes the following declaration as an application for pension under the provisions of the act of Congress approved May 1, 1920.
   That he is the identical Stephen M. Warner who enlisted August 10, 1862 at Mayesville, Kentucky, under the name of Stephen M. Warner, in Company B, 10th Regiment Kentucky Cavalry.  Discharged September 17, 1863, at Mayesville, Kentucky, having served the United States in the Civil War.
   That he also served in no other service and that otherwise than herein stated he was not employed in the United States military or service.
   That his personal description at the time of first enlistment was as follows: Height 5 feet 7 inches, complexion: light, color of eyes: blue, color of hair: black; that his occupation was farmer.
   That since leaving the service he has resided at in Bath County, now living in Mt.
Sterling, Montgomery County and his occupation has been a farmer.
   That he requires the regular personal aid and attendance of another person and has
required such aid and attendance since 1912 on account of the following disabilities:
rheumatism and dilated heart.  Total disability.
   That he did not serve in the Army, Navy or Marine Corps of the United States between April 6, 1917 and February 9, 1922, or at any time during that period.
   That no member of his family served in the Army, Navy or Marine Corps of the United States between April 6, 1917 and February 9, 1922, or at any time during that period.
   That his now a pensioner under Certificate No. 668926
