It's translation time again!

I transcribed this civil birth record from a document I copied.  I've been able to translate a good portion of it, but my dictionary does not have all the words and I think I have some of the words wrong.  Please take a look (click on the pictures below) then e-mail me and let me know what I have wrong or what I am missing.
