Prior to requesting a lookup, please be sure you have subscribed to the Family Search list. Instructions for subscribing to the list may be found on the main familysearch-L web page. You must be subscribed to the list for 90 days prior to requesting a lookup. This is to ensure that you understand the process involved and have tried to find other ways of obtaining the information you are looking for.
The Lookups Request form is for lookups related to restricted records only. If the record or film can be requested through your local FHC, do no ask for a lookup unless you have special circumstances listed below. Instead, go to your local FHC and request it there.
No more than 1 lookup request is allowed per month. Remember, we are volunteering our time to provide this service, and have jobs, families and lives apart from our genealogy work, as you do.
Special circumstances will be considered in the event someone needs a lookup that is not restricted. These circumstances are limited to those who live more than 100 miles from an FHC and those who, due to medical reasons, are unable to make a trip to their local FHC. Please describe your situation in the Additional Information box.
If you have special circumstances that are not listed above, and would still like to be considered for a lookup, please list them in the Additional Information box.
Costs related to the lookup may be incurred. These include the cost of film rental, photocopying and postage. The volunteer should contact the person requesting the lookup and arrange in advance payment of these costs. Costs should not exceed actual costs incurred. It is the Volunteers responsibility to collect costs prior to performing a lookup request.
Volunteers should choose one lookup at a time. Once that lookup is completed and the completed lookup form filled out, another request may be chosen.
