Sign-Up Process

Five forms are required to complete the sign-up process:

1. Application
2. Pedigree chart
3. Release of liability / liability waiver
4. Release of information
5. Family Tree DNA join request / order form

The following screens will step you through each of the forms. If you haven't already, you may want to print a checklist to more easily track your progress.

Important: Before signing up as a participant in the Kelly DNA study, please be sure you:

• read, understand and agree to the privacy policies of this site and of Family Tree DNA.

• understand what's involved with the test, how much it costs, and how we coordinate results.
