To J. T. Rippy and Sid Davis
Whereas all the proceedings necessary to entitle Mrs. Sina Black to be admitted
into the Tennessee Hospital for the insane as a non-paying patient have been
had according to law, you are hereby required forthwith to take said person
and convey her to said hospital and after executing this warrant, make due
return of the same to this office.

The undersigned a citizen of said state and county hereby states that Sina
Black, my wife, is insane. Her insanity is of less than two years duration
that her being at large is dangerous to the safety of the family and community
that she is in needy circumstances and has legal settlement in the said county
and is a citizen of the state of Tennessee.
These Facts can be proved by J. M. Graves and R. S. Pemberton M. D.

I R. S. Pemberton a practicing physician of the County of Sumner State of
Tennessee hereby certify that I have this day personally examined Sina Black
alleged to be insane and pronounced her so. She is about thirty eight
years of age. She is the subject of no infectious disorder. The
Disease is of about 12 or 14 months duration, is not subject of epilepsy
and I think her being at large is dangerous to herself and family and
disadvantageous to the community. She has not been treated for insanity.

State of Tennessee
Sumner County I the undersigned a Justice of the Peace
in and for the County aforesaid hereby certify that I have held an Inquest
on Sina Black of said County a person alleged to be insane according to Law.
I am satisfied that she is insane on competent testimony that she is
a fit subject to be sent to the State Hospital for the Insane to undergo
treatment therein as an object of the Bounty of the State. I am further
satisfied that her being at large is injurious to herself and disadvantageous
to the Community.
