To George House
Whereas all the proceedings necessary to entitle Mrs. Mollie House to be
admitted into the Tennessee Hospital for the insane as a non-paying patient
have been had according to law, you are hereby required forthwith to take
such person and convey her to said hospital and after executing this
warrant, make due return of the same to this office.
Witness my hand and Seal of Office at office this 5th day of Dec. 1887.

The undersigned a citizen of said state and county hereby states that Mrs.
Mollie House is insane, her insanity is of less than two years duration,
that she is on needy circumstances, and has legal settlement in the
said county and is a citizen of the state of Tennessee.
These Facts can be proved by A. J. Swaney M. D. and D. K. Spillers.

I, A. J. Swaney, a practicing physician of the County of Sumner State of
Tennessee hereby certify that I have this day personally examined Mrs. Mollie
House alleged to be insane and pronounced her so, is about (left blank) years
of age, is the subject of no infectious disorder. The disease is
of two weeks duration, is not subject of epilepsy, has never attempted to
commit violence on herself or others. The medical treatment has been
the force use of Bromide of Soda with Ferunginous Tonics & seclusion
with quiet as far as practical - sworn and subscribed to before me this 13th
day of Decr 1890.

State of Tennessee
Sumner County I the undersigned a Justice of the Peace
in and for the County aforesaid hereby certify that I have held an Inquest
on Mrs. Mollie House of said County a person alleged to be insane according
to Law. I am satisfied that she is insane on competent testimony that
she is a fit subject to be sent to the State Hospital for the Insane to undergo
treatment therein as an object of the Bounty of the State. I am further
satisfied that her being at large is injurious to herself and disadvantageous
to the Community.
