The hardcover book was published by Masthof Press with 869 pages, 7x10 inches, 60# white acid-free paper with black ink used throughout text, and Smyth-sewn in Roxite B-grade dark blue linen cloth with gold stamping on spine and cover. The book is well documented by footnote references & has an every name index.
